Description : Secondary to the hair cell loss there follows a progressive central neuronal dropout, which begins with the spiral ganglion cells of the modiolus and extends through the brain stem on to the higher centers.

Since the usual pattern of central auditory. neuronal dropout follows the tonotopic organization seen peripherally (beginning with the higher frequencies), the central auditory nerve loss is probably most often the result of the peripheral degeneration rather than of a central circulatory deficiency.
